High blood pressure (hypertension) happens when blood presses too hard inside your arteries. Over time, this can damage your heart, kidneys and blood vessels. Many people don’t know they have it.
Untreated high blood pressure raises the risk of heart attack and stroke. A reading near 120/80 is often considered normal. Your provider can help set your personal goal.
We’re here to help if you need support managing high blood pressure. You may be eligible if a doctor has diagnosed you with high blood pressure or another heart condition.

Diabetes can be hard to manage on your own. We’re here to help. We work with you and your provider to understand your needs and support you every step of the way.
If your diabetes is low risk, we’ll share tools and info to help you manage it. If it’s high risk, we’ll help you learn key skills, like:
Checking blood sugar
Taking medicine
Caring for your feet
Living healthier
You may qualify if you have diabetes or take diabetes medication.

Sickle cell disease is a blood condition some people are born with. It affects how red blood cells carry oxygen. Some cells become hard and sickle-shaped (like the letter “c”), blocking blood flow. This can cause pain, infections or other serious problems. Many people live longer today with early screening and better care. Support is here if you have sickle cell disease. Our care managers can help with doctors, appointments, rides and resources.

Childhood obesity means having extra body fat at a young age. Extra weight can raise the risk of diabetes and high blood pressure. It can also affect a child’s confidence and emotional health. Many factors play a role, including habits, genetics and hormones. Healthy meals and active routines can help the whole family. We’re here to help if your child needs support staying healthy.
Your child may qualify if a doctor says their weight is high for their age. They may also qualify if they need help with healthy eating or staying active.

Private duty nursing (PDN) offers in‑home medical care for children who have serious health challenges and need ongoing support. These services are a covered benefit for members under the age of 21 who need constant care. We cover, as medically needed, up to 24 hours per day with prior authorization (PA) or approval.

Your health matters — both physical and mental. Your plan covers care for the whole person, including mental health and substance use disorder services. Quitting nicotine or vaping can be hard, but help is here. You don’t have to do it alone. We offer support, tools and resources to help you quit for good.
If you’re pregnant and using nicotine, you can also get help with quitting. Check our pregnancy care page to learn more about quitting nicotine while pregnant. You can also talk with your doctor or contact us to get started.
