Care management

Our care managers are here for you. They can help you with your needs, goals and preferences, as well as coordinate care and services. Do you have a complex health condition? Our care managers understand your condition and can help you get the care you need. Not sure if care management is right for you? Complete your health risk screening and we’ll help you find out.

What does a care manager do?

We choose your care manager based on your needs. They’re your main point of contact and the person who helps coordinate your care. Care managers share info about your health conditions and ways to take care of yourself. They can also help you: 

 

  • Make a plan of care

  • Choose a primary care physician (PCP) 

  • Schedule doctor visits 

  • Find specialists and behavioral health services near home 

  • Get rides to your doctor, pharmacy or hospital 

  • Get medical and dental services, supplies and equipment 

  • Find community resources and educational materials 

  • Get the services you need

  • Manage your chronic conditions 

 

To help decide which services best meet your needs, the care manager works with:

 

  • You

  • Your providers

  • Your representative or guardian (if you have one)

  • Members of your care team that you choose

Care Connections Team (CCT)

We have a whole team behind your care manager. They work together to support your whole health. This includes your:

 

  • Physical health

  • Behavioral health (includes mental health)

  • Pharmacy access

 

In planning your care, we also keep other things in mind, like the conditions where you live, learn, work and play. These conditions can have a big effect on your health.

 

The CCT includes people who live and work in or near the communities they serve. Some team members include:
 

  • Local care managers, like yours
  • Clinical care managers who specialize in health topics like pregnancy and women’s health, behavioral health, substance use disorders, heart care and more
  • Community health workers
  • Community outreach staff

Aetna FamilyCare Central™

Aetna FamilyCare Central™

Aetna FamilyCare Central is your care coordination portal. You, your care manager and those in your Care Circle can log in to Aetna FamilyCare Central, get info and take action related to your plan.

 

  • Review documents like your care plans  

  • Look up your medicines

  • See your claims and prior authorizations (approval for services beforehand)

  • Show referrals to community organizations

  • See and update appointments 

  • Check recommended preventive and wellness care

  • Review your extra benefits

  • Order a new ID card

  • Message your care manager or Member Services 

  • Get notifications

  • Find your care manager’s contact info

 

Keeping all your health info together helps ensure we can align your goals with the services you get for your physical and behavioral health.

 

We use tools like Aetna FamilyCare Central to make sure you get the best care possible. It gives you and your care manager the info you need, when you need it.

Some health issues need more care. If you’re managing a chronic condition, a care manager can help you get the treatment you need to feel better. They can help you manage conditions like asthma, depression, diabetes, chronic obstructive pulmonary disease (COPD) and more.

Are you pregnant or planning to start a family? A care manager can help you stay healthy through your pregnancy. If your pregnancy is high risk, they can help you get the special care you need. You’ll get support, including info about preterm labor and birth.

Need help with the basics? SPPC helps people with the basics of daily living, like: 

 

  • Bathing and grooming 

  • Getting into and out of bed

  • Eating 

  • Shopping and errands

  • Housekeeping  

Complete your health risk screening

Once you join Aetna Better Health® of Oklahoma, we’ll call you for the screening. Or you can complete screening through your Member Portal. We'll use this info to better meet your health needs. Once you complete the screening, we’ll review your results and talk with you about your care options.
