Services

It’s hard to manage your health if you’re struggling with housing or having enough food to feed your family. Our Real Engagement And Community Help (REACH) Team wants to understand and help with your needs beyond medical care. We can connect you with programs that may help with:

  • Finances

  • Food

  • Education

  • Housing

  • Legal issues

  • Jobs

  • Support groups

  • Baby supplies

  • Clothing

All these topics are part of the social determinants of health. These things in life can make it harder or easier to stay healthy, like where you’re born, live, learn, work, play, worship, and age.

Call 1-833-316-7010 to get help from the REACH team. You can call Monday through Friday, 9 AM to 6 PM.

Need a quick answer to a health question? The 24-Hour Nurse Line is here for you. If it can’t wait until you see your doctor, call your plan’s Member Services number. Then, choose the option for the Nurse Line.
