Your doctor can help you choose the right one for you or your child.

If you have diabetes, we’re here to help.

Let your doctor and us know if you’ve been diagnosed. We’ll work with you to see if you’re at low or high risk and help you manage your care.

If you have Hepatitis C, our care managers will support you through your treatment.

They’ll help you stay on track with your medicine and teach you more about the disease.

Not sure if you need the ER? We can help.

Our Care Management team will guide you on when to call your doctor and when to go to the ER.

If you go to the hospital, a care manager will help you:

  • Make follow-up appointments
  • Get your medicine
  • Understand your discharge instructions
  • Arrange any other care you need

Before you leave the hospital, a care manager will help plan your next steps. They’ll help you move to another care place or safely return home.

If you may be near the end of life in the next year, we’ll help you and your family get special care that is kind and respectful.

If you have a long-term health problem, you’ll get your own care manager. They’ll call you after you join our plan and work with your doctors to make a care plan just for you.

Your care manager will talk with you by phone or visit you as often as needed.
