Food and nutrition service programs

As a provider, you play an important role in connecting members to the resources they need to manage and improve their health. For those eligible, we offer food and nutrition service programs at no extra cost. Learn more about our programs that help members maintain a healthy diet while managing a health condition.

Improving health through food and nutrition

 

Nutrition is key in managing and improving health conditions such as high blood pressure, diabetes, heart conditions and mental health issues. These programs are tailored to meet specific health needs, with input from registered dietitians, ensuring that patients receive the right nutritional support.

 

Our Programs

When eligible, members can use most programs for up to 6 months (or longer, if they have an approved medical need).

Who is eligible



Members may be eligible for any of these programs if they have a medical condition that makes it challenging for them to:

 

  • Get enough food when they need it

  • Shop for and cook healthy meals

How to qualify for each program

 

To qualify for this meal program, members must also: 

 

  • Live in regions 7, 8, 9 or 10 

    • Region 7: Clinton, Eaton and Ingham county
    • Region 8: Van Buren, Kalamazoo, Calhoun, Berrien, Cass, St. Joseph and Branch county 
    • Region 9: Livingston, Jackson, Washtenaw, Hillsdale, Lenawee and Monroe county 
    • Region 10: Oakland, Macomb and Wayne county 
  • Have a medical condition that can be improved with a healthy diet, such as 

    • High blood pressure, diabetes, gestational diabetes and other high-risk perinatal conditions, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), sickle cell disease, renal disease, cancer with malnutrition or human immunodeficiency virus (HIV) 
  • Have been in a hospital or skilled nursing facility within the last 60 days

To qualify for these meal programs, members must fit into one or more of these groups:

 

  • Live in regions 7, 8, 9 or 10 

    • Region 7: Clinton, Eaton and Ingham county  
    • Region 8: Van Buren, Kalamazoo, Calhoun, Berrien, Cass, St. Joseph and Branch county 
    • Region 9: Livingston, Jackson, Washtenaw, Hillsdale, Lenawee and Monroe county 
    • Region 10: Oakland, Macomb and Wayne county 
  • Have a medical condition that can be improved with a healthy diet, such as: 

    • High blood pressure, diabetes, malnutrition, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), sickle cell disease, renal disease, cancer with malnutrition or human immunodeficiency virus (HIV) 
  • Have been in a hospital or skilled nursing facility within the last 60 days 
  • Are pregnant and currently have, have a history of or are at risk of at least one of these conditions: 

    • High-risk pregnancy 
    • Delivery or birth complications, including: gestational diabetes, preeclampsia, newborn low birth weight, hyperemesis gravidarum and other causes of dehydration, maternal low birth rate of fewer than 2,500 grams, multiple pregnancy and malnutrition 
  • Used to be in foster care and has a nutrition-sensitive condition 
  • Are a child who has too much lead in their blood or has childhood obesity  
  • Are a child eligible for the Children’s Special Health Care Services (CSHCS) program
